Please print this one-page form, fill it in and bring it to your first SoftWave visit. It saves you a few minutes at the front desk.

What the form asks
Your name, date of birth, address, phone, email and an emergency contact. Then six yes or no questions that help us check SoftWave is safe for you:
- Have you had a cortisone shot this month?
- Do you have a heart pacemaker?
- Do you have cancer or a tumor?
- Do you have a skin infection?
- Are you pregnant, or might you be?
- Are you under 16?
If you answer yes to any of these, call us at 239.243.8810 before your visit so we can talk it through.
The last part is your consent to treatment, with your signature and the date.
